Overview of Second Quarter Performance

Smith and Nephew delivered underlying revenue expansion of 1.6 percent during the second quarter, a figure that fell short of earlier projections primarily because of subdued activity within United States orthopaedics alongside challenges in advanced wound bioactives. Despite this softer revenue trajectory, trading profit reached a solid 566 million dollars for the first half of the year, bolstered by 128 million dollars in operational efficiencies together with tariff refunds that neutralized anticipated tariff pressures. Management adjusted its full year revenue expectation downward to approximately 4 percent, citing ongoing softness in United States knee procedures and the SANTYL product line, yet the firm reaffirmed its commitments to profit targets, free cash flow, and return on invested capital.

Sports Medicine and ENT Segment Momentum

The Sports Medicine and ENT division posted robust underlying growth of 8.6 percent, fueled by double digit gains across joint repair offerings such as REGENETEN and Q FIX KNOTLESS. Performance remained consistent across geographic regions and product categories, with ENT products including FASTSEAL and related services also contributing meaningfully. In China, intentional inventory reductions ahead of volume based procurement implementation created some headwinds, yet overall demand outside that market stayed strong. Management highlighted continued adoption of REGENETEN, which expanded roughly 20 percent in the first half, underscoring its expanding role in rotator cuff repairs and additional tendon applications.

Advanced Wound Management Developments

Advanced Wound Management experienced a 2.1 percent decline on an underlying basis, driven largely by a 12.7 percent drop in bioactives revenue. Reimbursement modifications for skin substitutes under Centers for Medicare and Medicaid Services guidelines exerted pressure, particularly in nonsurgical and mobile settings where the company maintains limited presence. SANTYL faced additional friction from prior authorization requirements introduced by a major insurer, although underlying clinical demand stayed healthy. Management anticipates normalization for SANTYL in the second half and expects the skin substitute profit impact to reach the upper portion of the previously communicated 20 million to 40 million dollar range. Advanced Wound Care and Devices segments posted modest gains, supported by ALLEVYN COMPLETE CARE launches and strong emerging market performance for PICO and RENASYS.

Orthopaedics Segment Challenges and Outlook

United States orthopaedics revenue declined 1 percent on an underlying basis, reflecting persistent weakness in knees and temporary deployment delays for CATALYSTEM in hips. The shift toward cementless solutions continues to influence near term results, yet sequential improvement appeared through strong uptake of LEGION MS and double digit expansion in LEGION CONCELOC. Outside the United States, knees encountered a difficult comparison from a prior year Middle East tender, while hips benefited from the CATALYSTEM introduction in Japan. Trauma and extremities delivered solid overall growth, and the CORI robotic platform achieved double digit deployment increases globally. Management expects acceleration in the second half supported by the LANDMARK system launch and additional set deployments.

Financial Metrics and Efficiency Initiatives

For the first half, group revenue reached 3.1 billion dollars, reflecting 2.3 percent underlying growth. Gross margin expanded 60 basis points to 71.1 percent, aided by manufacturing and procurement efficiencies that more than offset inflation and inventory revaluation effects. Trading margin rose 60 basis points to 18.3 percent. Adjusted earnings per share increased 11 percent to 0.48 dollars, while the interim dividend grew 4 percent to 0.156 dollars per share. Efficiency savings reached 133 million dollars in the first half, prompting an upward revision of the full year target to 200 million dollars. Days sales of inventory improved substantially, particularly within orthopaedics, supporting better capital utilization.

Cash Flow, Balance Sheet, and Capital Allocation

Trading cash flow totaled 437 million dollars, with free cash flow at 231 million dollars after elevated capital expenditures tied to the Melton wound facility and information technology upgrades. Management reaffirmed approximately 800 million dollars in full year free cash flow expectations. Net debt stood at 3 billion dollars following the Integrity Orthopaedics acquisition and completion of 260 million dollars in share repurchases, resulting in a leverage ratio of 1.8 times adjusted earnings before interest, taxes, depreciation, and amortization. The company remains within its target leverage band around 2 times.

Updated Full Year Guidance and Strategic Priorities

Second half revenue growth is now projected between 5 and 5.5 percent, yielding full year underlying expansion near 4 percent. Profit, cash flow, and return on invested capital targets remain unchanged, supported by incremental efficiency gains and neutral tariff effects net of refunds. Key growth catalysts include continued REGENETEN and FASTSEAL momentum, LANDMARK system introduction, skin substitute market stabilization, and further CORI platform adoption. The acquisition of Integrity Orthopaedics continues to perform ahead of expectations, with integration focused on expanding shoulder repair capacity. Management emphasized ongoing execution of the RISE framework to address portfolio gaps and enhance resilience across all segments.
